| When applying for OSAP please have the following available:
- Social Insurance Number
- Date that you last attended High School on a full time basis
- Driver’s License number
- Make, model, year and license plate number, and current gross market value of all vehicles owned or leased by you (and your spouse if applicable)
- Information from partner or parents (if applicable)
- Date of birth-partner or parents (if applicable)
- Access to Income Tax Return-partner or parents (if applicable)
Supporting Documentation you will need to provide:
- If you are married: Copy of your marriage certificate
- You are in a Common Law or same-sex relationship: An affidavit signed by you and your partner confirming that you have been living together for at least 3 years or are raising any children by whom you are both the natural parents.
- You are a sole-support parent: Widowed or never married with a copy of death certificate (if widowed) and an affidavit confirming your children will be living with you on a full time basis during your study period and copy of your child tax benefit.
- You are separated, divorced or widowed and have no dependent children: A copy of your legal separation agreement, or divorce agreement.
- You are a permanent resident or protected person: Permanent resident with a Canadian Immigration Record or permanent resident card (both sides)
- Protected Persons: Photocopy of your valid protected persons status document (ppSD) and a photocopy of your temporary Social Insurance Number (SIN) card
