This is the complete application for the OSAP loan program.  This can be completed online or printed from this site and mailed.

This is a fact sheet that provides complete details on how to negotiate your loans.

2008-2009 OSAP Program Information Form
This program information form is for students planning to attend a postsecondary institution outside Ontario or Canada for a program that starts anytime between August 1, 2008 and July 31, 2009.

2008-2009 Supporting Documentation Form
Fill in the Student Information section and list all the supporting documentation you are required to provide.  Attach the documentation to this form.

Confirmation of Enrolment (Schedule 2)
This enrollment form is for borrowers and post-secondary institutions to complete as proof that a borrower is enrolled in school.

Continuation of Interest-Free Status/Confirmation of Enrolment (Schedule 2)
This Continuation Form is for students currently studying full-time and who are not receiving OSAP for the current period of study.

Tuition, Education, and Textbook Amounts Certificate form (T2202A).
